Nobody uses Falcons Eye nowadays, its outdated.
Use Vulture - http://www.darkarts.co.za/vulture/about - instead its in development still in 2011!
Version 3.31 is the latest build available through sourceforge or the website given as a homepage and download location.
The application homepage does mention a possibility of using version 3.40 of nethack with some effort.

Note: you may able to use even the current version of Falcon's Eye with NetHack 3.4.0. This may require minor additions to NetHack 3.4.0 source files. Had the same problem (also running XP)...don't know if it was because I retried nethack.exe a few times or because I renamed the app folder "nethack", but it began working consistently afterwards...

Falcon's Eye hasn't been updated in years...I recommend you use Vulture's Eye, a fork of the code:

+1 for Gadren & Markus: Vulture is now v2.6.3 (last updated on 2012-10-27, according to https://bitbucket.org/clivecrous/vulture), and has downloads for Linux and MacOS, as well as for Windows.
