The latest version is an installer, not the self-contained EXE described in the entry http://www.portablefreeware.com/index.php?id=418
There's been a lot of additions to the program since it was first posted: http://www.diskcleaner.nl/article/cat/news
Edit: Tested with 7zip and Universal Extract without success.
The author seems to understand the program staying portable, but seems to have gone a different direction since a post addressing it.

It's an NSIS installer and UE works OK for me.
This app is portable if you start it using the "-p" switch!
This is true for both versions: 1.7.1645 (stable) and 1.8.1795 (RC4).

UE works fine.
Run dclean.exe with the -p command line switch. It will create dcsettings.ini file in the application folder and will not write in %APPDATA%
Tested with Disk Cleaner version 1.7.1645.8960 on XPSP3 machine.

Tested V1.8.1795: Portable (Not Stealth)
It created HKCU\Software\dclean and an INI file in the application folder (not %APPDATA%) and I used the -p commandline switch as suggested.
I see what you mean with the checkboxes, they are black until you re-select them.

No registry entries on my test. Even without "-p" switch. Just the .ini file.
And the GUI looks OK for me.

It's sounded suspicious so I tested again...the tests were created one after the other and the system wasn't started from scratch, but it was tested on a machine that hadn't had Disk Cleaner installed before.
Test 1: Opened dclean.exe -p and Closed again, only dcsettings.ini was created and no registry entries. Portable
Test 2: Did the same as Test 1 but whilst the application was open, I changed settings in the options dialogue box. Portable
Test 3: Did the same as Test 1 but whilst the application was open, I selected the "Clean" button and then selected "Back" once it had finished. This created the Registry Key. Not Portable
Test 4: Did the same as Test 1 because I wanted to see if the Registry Key was deleted on re-launch, it wasn't! Not Portable
Reverted back to an earlier snapshot (VirtualBox) and re-did Test 3 but instead of selecting "Back" I chose "Exit" up completion of cleaning, as this was the Test that created the Registry Key. It didn't! Portable
Conclusion: Exit the program after cleaning!
